My head is a 7-1/4. Usually, that is a large, but in this case, the medium fit better. My son is a 7-3/8 with too much hair, and it works for him.

This is a wonderful, warm cap. However, even tho' I measured my head according to the instructions and chose the size via their chart, it still was too large. I've solved that by wearing it over a knit skull cap, since it's cold outside when I wear it anyway. On a recent visit to a cold NYC, it was perfect.
